Find Care — Project Kos
Find Care

Find the right provider
and the next right direction.

CMS-backed facilities and Medicare-enrolled suppliers — found, explained, and routed. When the answer lives elsewhere, we say so and point you there.

Live here: hospitals, nursing homes, home health, hospice, dialysis, and equipment suppliers from federal CMS data. Routed out: VA, IHS, state Medicaid, and plan-specific tools.

How this works
What this tool does — and why location matters
We require your location because CMS has data on every Medicare-certified provider in the country. Without a zip code or city, results are meaningless — you need to know what is actually reachable.

What you get: Every result comes from federal CMS data — the same database hospitals and discharge planners use. Ratings reflect unannounced inspections, staffing levels, and patient outcome measures. Red flags (Special Focus, abuse indicators, federal fines) are surfaced so you can ask the right questions.

What this cannot do: It cannot verify insurance network status, confirm bed availability, or replace a referral conversation. Use it to narrow options and go into conversations with providers informed.

Outside CMS

Veterans Affairs

VA does not fit cleanly into CMS-only searches. Use VA's own finder, then return here for CMS-backed comparisons.

Outside CMS

Indian Health Service

IHS and tribal health systems should be searched through official IHS resources, not forced through CMS filters.

State by state

Medicaid

Medicaid varies by state. Use the official state path for waiver services, long-term supports, or Medicaid-specific providers.

Plan-specific

Dental, vision, hearing

These benefits depend on Medicare Advantage or other plan-specific networks. This page explains the system and routes you to the right finder.

Medicare typically covers
  • Inpatient hospital stays Part A
  • Skilled nursing rehab up to 100 days Part A
  • Inpatient rehabilitation Part A
  • Home health Part A/B
  • Hospice Part A
  • Dialysis for ESRD at any age Part B
  • Durable medical equipment Part B
Skilled nursing and home health depend on eligibility and medical necessity, not just the setting.
Generally not covered
  • Long-term custodial nursing home care
  • Assisted living room and board
  • 24-hour home care
  • Homemaker or personal care only
  • Most dental, vision, and hearing
  • Routine eyeglasses and hearing aids
Gaps may be filled by Medicaid, Medicare Advantage, long-term care insurance, or veteran benefits.
