Federal law guarantees every Medicare beneficiary the right to choose any qualified, enrolled provider. At discharge, hospitals are required to offer you a choice of agencies — not just the one they partner with. Most referrals are shaped by contracts and relationships you can't see. Unlike Medicare, Medicaid is a joint federal-state program. Every state sets its own eligibility rules, covered services, and waiver programs. A single list would be misleading.
Common gap-filling roles Medicaid plays include long-term nursing home care, home and community-based services (HCBS) waivers, personal care, and Medicare premium/cost-share assistance through Medicare Savings Programs.
