* Requires a 3-day qualifying inpatient hospital stay. Days 1-20 fully covered; days 21-100 have a $217/day coinsurance in 2026; day 101+ not covered.

Must be homebound, require intermittent skilled care (generally under 8 hours/day, 28 hours/week), and use a Medicare-certified agency.

Requires physician certification of a terminal illness with 6 months or less prognosis. Electing hospice waives curative treatment for the terminal condition.

§ Medicare pays 80% of the approved amount after the $283 Part B deductible (2026); you pay 20%. Only applies when using a Medicare-enrolled supplier who accepts assignment.

Medicare generally doesn't cover
  • Long-term custodial nursing home care
  • Assisted living room and board
  • 24-hour home care
  • Homemaker or personal-care-only services
  • Most dental, vision, and hearing care
  • Routine eyeglasses and hearing aids
  • Cosmetic procedures
  • Care outside the U.S. (with rare exceptions)
These gaps are often filled by Medigap, Medicare Advantage extras, long-term care insurance, or Medicaid.
Medicaid coverage varies widely by state

Unlike Medicare, Medicaid is a joint federal-state program. Every state sets its own eligibility rules, covered services, and waiver programs. A single list would be misleading.

Common gap-filling roles Medicaid plays include long-term nursing home care, home and community-based services (HCBS) waivers, personal care, and Medicare premium/cost-share assistance through Medicare Savings Programs.
